PDO Threads or Dermal Fillers: Which Is Right for You?
Understanding Which Treatment Best Matches Your Goals
One of the most common questions I hear from patients interested in facial rejuvenation isn’t about Botox®, dermal fillers, or PDO threads individually.
It’s much simpler.
“Which treatment do I actually need?”
It’s an excellent question, and one that deserves a thoughtful answer. Both dermal fillers and Smooth PDO Threads are used in nonsurgical facial aesthetics, but they work differently and address different concerns. One primarily restores volume, while the other is intended to encourage collagen remodeling within the skin. Neither is automatically the better choice.
After more than 25 years practicing dentistry in Henderson, Nevada, I’ve learned that successful facial aesthetics begins with understanding the person. Not selecting a product. At my practice, every consultation starts with evaluating facial anatomy, listening to your concerns, and discussing what can realistically be achieved.
Because beautiful facial rejuvenation isn’t about adding more treatments. It’s about understanding what your face needs—and sometimes recognizing what it doesn’t.
Why Does Our Facial Appearance Change With Age?
Before deciding between dermal fillers and PDO threads, it’s important to understand why our faces change over time.
Facial aging involves several overlapping processes:
- Volume loss: Changes in facial fat compartments can create hollowing or flattening.
- Collagen and elastin changes: Skin gradually loses some of its firmness and elasticity.
- Bone remodeling: Changes in underlying facial bone can affect structural support.
- Muscle movement: Repeated facial expressions contribute to dynamic lines.
- Skin changes: Sun exposure, genetics, and lifestyle factors influence texture, pigmentation, and fine lines.
- Dental changes: Tooth loss, wear, and changes in bite support can influence the appearance of the lower face.
These changes don’t occur equally in every person. One patient may be concerned about thinning lips. Another may notice fine lines around the mouth. Someone else may experience hollowing in the cheeks or more noticeable skin laxity.
Although these concerns can appear similar, they may require very different approaches.
The first question isn’t which treatment is popular. It’s what is actually causing the concern.
What Are Dermal Fillers?
Hyaluronic acid dermal fillers, including products within the Juvéderm® family, are injectable gels designed to restore or enhance volume in specific areas of the face.
Hyaluronic acid is a substance naturally present in the body that helps retain moisture. When formulated as a dermal filler, it can provide structural support and improve selected facial contours.
Depending on the specific product, its approved indications, and the patient’s anatomy, fillers may be used to address:
- Thinning lips
- Certain facial folds and wrinkles
- Loss of cheek volume
- Selected chin and jawline contour concerns
- Facial asymmetry
- Age-related volume changes
One advantage of hyaluronic acid fillers is that changes in volume are often visible immediately, although swelling and bruising may temporarily affect the appearance. Results generally settle as the initial swelling resolves.
What Dermal Fillers Cannot Do
Fillers are not a universal solution for facial aging. They cannot reliably correct significant skin laxity, eliminate every wrinkle, or replace surgical treatment when substantial tissue repositioning is needed. Adding too much filler may create unwanted fullness or distort natural facial proportions.
That’s why I prefer a conservative approach. The goal isn’t necessarily fuller lips or larger cheeks.
It’s balanced facial proportions that still look like you.
What Are Smooth PDO Threads?
Smooth PDO Threads are fine, absorbable threads made from polydioxanone, a material also used in certain medical sutures. Unlike dermal fillers, smooth threads do not create meaningful immediate volume. Instead, their placement is intended to stimulate a localized tissue response, including collagen remodeling, as the threads gradually break down.
They are sometimes used for selected concerns involving:
- Fine lines
- Mild skin texture changes
- Early signs of skin aging
- Subtle changes in skin firmness
- Areas where gradual improvement is desired
However, it’s important to understand that the clinical evidence for predictable cosmetic improvement from Smooth PDO Threads remains limited. Some patients may notice subtle changes over time, while others may experience little visible improvement.
Results depend on treatment technique, individual tissue response, the area treated, and other factors. I believe patients deserve to understand both the potential benefits and the limitations before deciding whether treatment is worthwhile.
Smooth PDO Threads Are Not the Same as PDO Thread Lifts
This distinction is especially important because social media frequently groups all PDO thread procedures together.
Smooth PDO Threads are generally used with the intention of encouraging collagen remodeling. They do not have the mechanical anchoring structures needed to produce a substantial lifting effect.
Barbed or cogged PDO threads, commonly associated with thread-lift procedures, are designed to engage tissue and provide some degree of mechanical repositioning.
These are different techniques with different goals and risks. Neither should be considered equivalent to a surgical facelift.
At my Henderson practice, the focus of Smooth PDO Thread treatment is subtle skin-quality enhancement in appropriately selected patients. Not dramatic lifting or facial transformation.
PDO Threads vs. Dermal Fillers: Understanding the Differences
| Feature | Smooth PDO Threads | Hyaluronic Acid Dermal Fillers |
|---|---|---|
| Primary purpose | Intended collagen stimulation | Restore or enhance volume |
| Immediate volume | Minimal to none | Often visible immediately |
| Results | Gradual and variable | Immediate, then settles |
| Potential applications | Selected fine lines and skin-quality concerns | Volume loss, folds, lips, contours |
| Duration | Variable; limited long-term evidence | Depends on product and treatment area |
| Main limitations | Subtle or unpredictable results | Cannot reliably correct substantial laxity |
| Common side effects | Bruising, swelling, tenderness | Bruising, swelling, tenderness |
The simplest way to understand the difference is:
Fillers primarily add volume. Smooth PDO Threads aim to stimulate a tissue response.
That doesn’t mean one is better than the other. It means they serve different purposes.
Three Questions I Ask Before Recommending Treatment
When a patient visits my office for facial rejuvenation, I consider three important questions.
1. Is the Concern Volume Loss, Skin Quality, or Tissue Laxity?
These are not interchangeable problems.
If a patient has lost volume in an appropriate treatment area, a carefully selected dermal filler may be helpful.
If the primary concern is fine lines or mild skin-quality changes, other treatments may be worth considering, including Smooth PDO Threads, depending on the patient’s expectations and clinical findings.
If significant tissue laxity is present, neither filler nor smooth threads may provide the desired improvement.
Identifying the underlying concern helps avoid unnecessary treatment.
2. What Degree of Improvement Is Realistically Possible?
Patients sometimes arrive with photographs of dramatic facial transformations.
I welcome those conversations because they help me understand what someone hopes to achieve. But I also explain what a particular treatment can and cannot accomplish.
A subtle improvement may be possible without surgery.
A dramatic lift generally is not.
Realistic expectations are an essential part of patient satisfaction.
3. Would a Different Treatment or No Treatment Be Better?
Not every aesthetic concern requires an injectable procedure.
Some patients may benefit more from:
- Botox® Cosmetic for selected expression lines
- Professional skincare and sun protection
- Cosmetic dental treatment to address tooth wear or smile proportions
- Orthodontic treatment for appropriate alignment concerns
- Another medical or aesthetic evaluation
- Simply allowing time before pursuing additional treatment
Sometimes my recommendation is to do less. And sometimes it’s to do nothing at all.
The best treatment plan is not necessarily the one with the most procedures.
Can PDO Threads and Dermal Fillers Be Combined?
In selected patients, combining different facial aesthetic treatments may be appropriate.
For example, a patient may have volume loss in one area and separate skin-quality concerns in another. A dermal filler might address the volume concern, while another treatment could be considered for the skin. However, combining procedures doesn’t automatically produce better results.
The decision depends on:
- Facial anatomy
- Treatment locations
- Skin condition
- Medical history
- Product selection
- Potential complications
- Individual goals
In some situations, treatments may be performed during the same visit. In others, spacing appointments may be more appropriate. I prefer to develop a thoughtful treatment sequence rather than treating every concern at once.
Are PDO Threads and Dermal Fillers Safe?
Both procedures are minimally invasive, but neither is risk-free. Understanding possible complications is an important part of informed consent.
Dermal Filler Risks
Common temporary effects include swelling, bruising, tenderness, and redness. Other complications can include lumps, asymmetry, infection, delayed inflammatory reactions, and unwanted product placement.
Rare but serious complications can occur if filler enters or compresses a blood vessel, potentially causing tissue injury, skin necrosis, or vision loss. These complications require urgent medical assessment.
Smooth PDO Thread Risks
Potential complications include:
- Bruising and swelling
- Tenderness or discomfort
- Infection
- Visible or palpable threads
- Thread migration or extrusion
- Irregularities or asymmetry
- Inflammatory reactions
Not everyone is an appropriate candidate for these procedures. A thorough medical history, anatomical evaluation, and discussion of risks are essential.
The goal is never simply to perform a treatment. It’s to determine whether the potential benefit justifies the risks for that individual patient.
Which Treatment Lasts Longer?
This is another common question, but there isn’t one answer that applies to everyone.
Hyaluronic acid filler results commonly last several months to more than a year, depending on the specific product, treatment area, injection technique, and individual characteristics. Some products may provide longer-lasting effects in their approved treatment areas.
Smooth PDO Threads gradually dissolve over time. Although tissue remodeling may occur, the magnitude and duration of visible cosmetic improvement are variable, and robust long-term evidence is limited.
Neither treatment should be considered permanent.
Patients should also understand that maintaining a particular aesthetic result may require future treatment, with additional costs and risks. I encourage patients to think about long-term maintenance before beginning any elective facial aesthetic procedure.
Facial Harmony: Why My Background in Dentistry Matters
One aspect of facial aesthetics that I find especially important is the relationship between the smile and the surrounding face.
As a cosmetic and restorative dentist, I spend much of my professional life evaluating the teeth, lips, bite, and facial proportions together.
The teeth help support the lips.
The bite influences how the jaws relate.
Smile width, tooth length, and gum display affect the way facial features are perceived.
These relationships become especially important when evaluating the lower third of the face.
Sometimes a patient is concerned about the appearance of the lips or mouth, but part of the concern may involve worn teeth, missing teeth, or aging dental restorations. In those situations, adding filler may not address the underlying issue.
A dental evaluation can sometimes reveal options that are more appropriate. That doesn’t mean everyone considering facial aesthetics needs cosmetic dentistry. It simply means the smile and the surrounding facial structures deserve to be evaluated together when relevant.
Why I Believe Less Often Creates More
One of the most common misconceptions about facial rejuvenation is that more treatment creates a better result.
I don’t believe that’s true.
In fact, subtle improvements can be more attractive than dramatic changes because they preserve the facial features that make someone recognizable. When evaluating treatment, I consider:
- Natural facial proportions
- Lip movement and expression
- Facial symmetry
- Skin quality
- Existing dental support
- Individual preferences
- Long-term maintenance
I don’t want patients to feel that they need to keep adding products to maintain their confidence. I want them to feel comfortable with the decisions they make.
Sometimes that means using less filler.
Sometimes it means recommending a different approach.
And sometimes it means explaining that the expected improvement may be too small to justify treatment.
My philosophy is to enhance the individual. Not create an entirely different face.
What to Expect During a Facial Aesthetics Consultation
A successful consultation begins with listening. Before recommending treatment, I want to understand what you’re seeing when you look in the mirror.
What concerns you most?
When did you first notice the change?
Are you looking for a subtle improvement or something more noticeable?
Have you had previous aesthetic treatments?
What are your expectations for recovery, maintenance, and cost?
We also review relevant medical history, medications, previous procedures, and any factors that may affect treatment suitability. Then we discuss the available options, including the possibility that a particular treatment may not be appropriate.
Patients should leave a consultation understanding not only what might help, but also what the limitations and risks are. That is what informed, personalized care means to me.
Frequently Asked Questions
How do I know whether I need Smooth PDO Threads or dermal fillers?
The best choice depends on the underlying concern. Hyaluronic acid fillers are primarily used for selected volume and contour concerns. Smooth PDO Threads are intended to stimulate tissue remodeling, although visible benefits are less predictable. An in-person evaluation is necessary to determine whether either treatment is appropriate.
Can Smooth PDO Threads replace dermal fillers?
Generally, no. Smooth PDO Threads do not provide the immediate volumizing effect of hyaluronic acid fillers. They are different procedures with different treatment goals.
Can I have PDO Threads and fillers during the same appointment?
Possibly. Some patients may be candidates for combined treatment, but timing depends on anatomy, treatment locations, medical history, and clinical judgment. Separate appointments may sometimes be preferable.
Which treatment lasts longer?
Hyaluronic acid filler longevity varies by product and treatment area. Smooth PDO Threads dissolve gradually, but the duration of any visible cosmetic improvement is less predictable. Neither treatment offers permanent results.
Are Smooth PDO Threads the same as a nonsurgical facelift?
No. Smooth PDO Threads do not provide the mechanical lifting associated with barbed lifting threads and cannot reproduce the effects of surgical facial rejuvenation.
Are dermal fillers reversible?
Hyaluronic acid fillers can sometimes be reduced or dissolved using hyaluronidase. However, reversal is a medical procedure with its own considerations and does not guarantee correction of every complication. Smooth PDO Threads cannot be dissolved in the same way.
Will my results look natural?
My goal is always to achieve subtle, natural-looking results. Conservative planning and careful attention to facial anatomy can help, but outcomes vary, and no aesthetic procedure can guarantee a specific appearance.
How much do PDO Threads and dermal fillers cost in Henderson, NV?
Costs vary based on the product or procedure, treatment areas, complexity, and amount of treatment required. During a consultation, I can explain the recommended approach, anticipated fees, and possible future maintenance needs.
Looking Like Yourself Never Goes Out of Style
The most rewarding part of facial aesthetics isn’t simply seeing a wrinkle soften or a contour change. It’s helping someone feel more comfortable with their appearance while preserving what makes them unique.
I believe facial rejuvenation should be thoughtful, conservative, and based on realistic expectations.
Not every patient needs dermal fillers.
Not every patient is a candidate for Smooth PDO Threads.
And not every sign of aging needs to be treated.
Sometimes the most important part of my job is explaining why a particular treatment may not be the right choice.
At my Henderson, Nevada practice, serving Green Valley, Anthem, Seven Hills, Summerlin, Inspirada, and the greater Las Vegas Valley, I approach facial aesthetics with the same philosophy that guides my cosmetic dentistry:
Listen first. Evaluate carefully. Recommend thoughtfully. Preserve individuality.
Because beautiful results aren’t about looking like someone else. They’re about feeling confident in the face you already have.
