The standard of care for professional dental cleanings has long been every 6 months. This still holds true today for most patients. Studies have repeatedly shown that those who goto regular dental visits are less likely to have the need for a dental restoration or to have a tooth removed.

Are Teeth Cleanings Necessary?

While for many at home care can be a breeze for many others it is a constant struggle. Our daily lives can get in the way of even the simplest tasks including our oral hygiene. Regular professional teeth cleaning removes plaque, the soft yellowish build-up, and calculus (hardened plaque) that we are just not able to get to. This soft build-up is made up of billions of different types of bacteria that live and reproduce in our mouth by feeding on the food we eat releasing acids that if left alone will damage our teeth and periodontal tissues.

Most bacteria co exist in our bodies without causing too much trouble to our health and well being. But certain bacteria in dental plaque, when they grow in numbers, can lead to tooth decay or periodontal disease.

A professional dental cleaning will reduce your chances of developing tooth decay or periodontal disease by significantly reducing the amount of plaque, calculus, and bacteria in your mouth.

How Often Is Acceptable?

As a dentist, my patients often ask me how regularly they should come in to get their teeth cleaned. My response is usually: “That depends”. For most of the population every 6 months is the right number. But there are those who just cannot maintain dental hygiene on their own or they develop calculus faster than others. So for those patients I will recommend a schedule of every 3 months. 2 extra visits a year to maintain your dental health should not feel like a lot. This will reduce chances of tooth decay and periodontal disease development in these patients over the long haul.

Factors Affecting Dental Hygiene

We know certain lifestyle choices can affect a person’s risk of developing tooth decay and periodontal disease. Following are some questions you may ask yourself to understand whether you are at an increased risk:

-Does your drinking water contain Fluoride? Is this your main source of drinking water?

-Do you frequently snack, including on sweets?

-Are you a regular flosser?

-Do you brush your teeth at least twice a day?

-Do you visit your dentist for toothaches rather than routine examinations?

-Have you had multiple teeth with tooth decay at your last few dental visits?

-Is your dentist “watching” a lot of teeth with early tooth decay?

-Do you have to wear a denture or undergoing orthodontic treatment?

-Do you develop excessive amounts of calculus quickly?

-Do you suffer from a chronic long-term health condition such as diabetes?

-Do you suffer from a xerostomia (dry mouth)?

If you were able to answer “yes” to most of the questions above, you are likely to need to see your dentist or hygienist at least every six months, if not more often based on your dentist’s recommendations. Following a professional cleaning, people prone to tooth decay can benefit from the fluoride treatment following removal of plaque and calculus. Studies have shown that professional fluoride treatment every six months can lead to about a 30% reduction in the development of tooth decay.

Dental Health = Overall Health

Some patients with chronic health issues such as cardiovascular disease or diabetes will need to see their dentists more frequently. This is because they are much more susceptible to periodontal disease.

Patients taking blood thinners and other medications, such as for osteoporosis, may need to visit the dentist more regularly as well. At home dental hygiene is often neglected due to our busy schedules. While many are diligent in maintaining good oral health at home others let it slide. The recommended daily regimen is brushing in the morning after breakfast and again before bedtime. Is it helpful to brush more often?

Tooth Brushing Frequency

The answer to the above question is YES and NO! It is most definitely possible to over brush our teeth. It is estimated that approximately 80% of us are over brushing. Some of us become obsessed, but we are brushing incorrectly and we are brushing with the wrong type of toothbrush. There is definitely such a thing as too much of a good thing when it comes to brushing your teeth.

Brushing your teeth too frequently and too aggressively can damage the structure of our teeth as well as wear out the tooth enamel. Toothbrushes are designed to last around 3-4 months before they lose their soft bristles. However, frequent usage of the toothbrush would wear down the bristles and leave the bristles jagged and sharp, with serrated edges. When this happens, your toothbrush can then wear away the enamel and dentin from our teeth.

As a result, you should replace your toothbrushes before the bristles splay so that you’re reassured that your toothbrush would not wear away your tooth structure. You also should choose toothbrushes that will not hurt your gums and teeth. It is recommended that a soft bristle toothbrush is used unless recommended differently by your dentist.

Proper Brushing Technique

1. You should always try to choose a toothbrush with soft bristles. Tooth brushes can be chosen on individual preference.  Basically, choose one that you are comfortable with.  Also size of brush head is important. If the toothbrush is too large to fit properly in your mouth, you will be unable to reach the further most areas. For many an electric toothbrush will work best. Our office recommends the Rotadent (made by Zila) electric toothbrush but any would be fine if it is effective for you.

2. Rinse your brush before using it. A small amount of tooth paste is enough for brushing. You do not need to cover all  the bristles with toothpaste. Just want enough on there for the paste to do its work.

3. Hold the tooth brush angulated at 45 degrees to your teeth towards the  gums. Do not hold the brush flatly against your teeth. You want to be able to have the brush go all the way down to the gum line where most of the plaque resides. Paying extra attention to the gum line, hard-to-reach back teeth and areas around fillings, crowns or other restoration.

4. Start from the back of the upper jaw. You need to ensure you are cleaning all surfaces of the teeth effectively. There are three surfaces…..

1. The chewing surface of teeth.

2. The outer surface of the teeth facing the cheek.

3. Inner surfaces of the teeth.

Start with 3 teeth at a Clean on all 3 surfaces of the teeth. Use gentle pressure with short back and forth movements. Avoid using vigorous pressure as it is unnecessary. Let the brush and paste do the work for you.

5. After cleaning the 3 back most teeth, move on the next 3 teeth…..and so on.

6. After you have completed brushing your upper teeth, move on to your lower teeth and complete brushing in the same way as the upper teeth.

7. After brushing your teeth, brush along the gum line gently to remove any lodged food particles. Also, brush your tongue. Brushing your tongue will remove bacteria and buildup there to help with bad breath.

Tooth Brushing Recommendations

1. It is necessary to brush your teeth at least twice a day. Most people do this once every morning upon waking up, and before they go to bed at night. You can also brush your teeth after your midday meal or snacks.

2. Use short, gentle strokes to clean your teeth. Brush the outer and inner surfaces, as well as your tongue. If you have to clean in between your teeth, then use vertical brush strokes.
Then, you should brush your teeth for at least two minutes at a time. This would ensure that you have adequately removed all the plaque, food residue, and bacteria that may be in your mouth.

3. Remember to floss each night before going to bed to ensure that there is nothing stuck in your teeth.

One last tip, would be the use of a plaque disclosing solution. This would give you a visual aid to see what areas you are missing or need work on.

Toothbrushing Can Keep Away Tooth Decay

Above all, you should visit your dentist every six months for a professional dental cleaning to remove any tartar or plaque build up. These regular visits would ensure that your mouth is as healthy as it can be. Your dentist would also use this opportunity to clean your mouth thoroughly to get rid of the tartar and plaque buildup.

With the proper toothbrush, brushing techniques, and regular visits to your dentists, you are on your way to having a perfect, flawless smile. Just remember to regularly brush your teeth – and to brush them for at least two minutes!

Dental Tips For Moms

Drink Lots Of Water. Drinking water is not only good for our bodies but also for our teeth. We tend to snack more when we are going from different events with little to no time to worry about our dental health. Drinking water will keep you and your kids hydrated but it will also reduce the effects of frequent snacking on your teeth. Rinsing with water following snacking will help reduce the food trapped between our teeth as well as reduce the acidic levels which lead to tooth decay. Flavored waters and gatorade are loaded with sugar, so stay away from sugary beverages.

Travel Toothbrushes And Floss. These compact brushes and floss picks are great for a quick toothbrushing and flossing in the back of the car before going to a new event. They will not only keep your teeth clean but they can also freshen your breath. Many travel toothbrushes already have toothpaste on them. Wetting the brush will release the toothpaste.

Snack Wisely. Not all snacks are created equally when it comes to our teeth. Snacks high in sugar and carbohydrates are worse for our teeth over the course of a long day. Choose snacks like fresh fruits, Not “fruit snacks”, (which tend to stick and stay between the teeth ) raw vegetables, lentil chips, edamame, milk, cheese, granola, whole wheat crackers, and nuts. You can even make quick oatmeal cookies by mixing an overripe banana with plain oatmeal or flavored instant oatmeal and placing spoonfuls on a cookie sheet with parchment paper, bake at 325 about 20 minutes. These can be eaten as a meal or a snack.

Drink Sugary Drinks Through A Straw. Due to the sugary deposits left in the mouth, using a straw while drinking the soda will have less contact with the teeth. Of course, it is better to avoid sugary beverages entirely, but if you must do them, use precautions and drink water directly after.

Chew Gum. Pack sugar free gum that contains Xylitol (sugar substitute) to help kill bacteria in the mouth after eating during the day. Gum will also help pull food particles off the teeth and stimulate saliva flow to help neutralize acids.

Keep A Set Of Toothbrushes By The Kitchen Sink. When mealtime needs to be rushed, most kids will not go back to the bathroom to go and brush. If the toothbrushes are right there when the dishes are going in the sink, they are more likely to find the time to brush and rinse after the meal.

Use Fluoride Rinses. Be sure to have children rinse with a fluoride rinse before bed, this should help strengthen teeth and make cavities less likely to happen.

Prepack Your Fridge. If you have pre-made sandwiches and healthy snacks like washed grapes, apple slices, or cheese cubes in ziploc bags, it is easy to grab what you need and not have to stop for unhealthy fast foods, donuts, and snacks.

Educate your kids. When kids learn about making healthier choices, have them readily available, and start to enjoy the bag of fresh berries over the flaming hot cheetos they will start feeling better and enjoy a more healthy lifestyle.

Don’t eat gummies! For some reason, packaged processed “fruit snacks” have become a new food group. Parents are under the false impression that they are healthy. They do not take the place of fresh fruit, and are one of the leading causes of tooth decay in toddlers and children. Remember, natural cane sugar is still sugar. When it sticks and stays between your child’s teeth, it will cause decay.

It all starts with brushing at least twice a day with a fluoride toothpaste and flossing at least once per day. Consistency is the key to maintaining good dental hygiene.

Best Dental Products

1) Automatic Toothbrush. An automatic toothbrush is able to reach places that most manual toothbrushes are unable to. Another advantage of an automatic toothbrush is that they help prevent wear that aggressive manual brushing causes. Most automatic toothbrushes have a timer on them. This ensures that you are brushing for the proper amount of time. An automatic toothbrush is especially helpful for patients with limited dexterity either from age or disease. There are two types of automatic toothbrushes:

UltraSonic Toothbrushes. These toothbrushes can generate between 30,000 and 40,000 brush strokes per minute. By comparison, manual toothbrushing can only generate 300 brush strokes per minute. The bristles also rotate in the recommended back and forth motion needed to clean teeth and gums properly. Another plus of these sonic toothbrushes is that they have a patented cleaning action that actually directs fluid between teeth and below the gum line to gently remove plaque and food debris.

Electric toothbrushes. These toothbrushes can generate between 3,000 and 7,500 brush strokes per minute. Generally these brushes work by rotating in a circular motion. Some even have a dual action with pulsating motion added in as well to facilitate plaque and food debris removal.

Although individual designs differ, the bristles in the brush head are typically either set in a circular format that rotates (the entire head rotates in unison) or individual tufts of bristles within the brush head spin independently. Some electric toothbrushes have both a rotating as well as a pulsating motion to help remove plaque and reduce gingivitis.

2) Water Irrigators with Periogen. the most common being the waterpik ultra. Most people do need these to aid in dental hygiene (especially those with bridgework, implants, and braces). The irrigation helps to stimulate blood flow to heal gum tissue, remove hard to reach food particles, and get up to 6mm into a periodontal pocket. Recently a new product has come to market called Periogen. Periogen is a unique product in that it is an oral rinse that removes tartar buildup between professional cleanings. The waterpik aids in getting the periogen oral rinse below the gum line and in between teeth.

3) Disclosing Solution. This is generally used for kids but can apply to adults having trouble keeping their teeth clean. The disclosing solution is used as a rinse and it will color the plaque and food debris on your teeth making what you normally miss during brushing very visible. You are able to then remove  any residue of color remaining and know what you need to do next time. Its a great visual tool to really make a difference in your oral health.

4) Flossers. While there are many new flosses that do a great job (such as reach total care), many patients do not floss regularly. Luckily, a product has been developed called a flosser( such as reach access flosser). Makes cleaning between teeth as easy as can be.  These are also helpful for those with limited dexterity as well as children.

5) Tongue Scrapers (also called tongue cleaner). This will allow you to remove food debris, fungi, dead cells and bacteria that builds up on the tongue. The benefit of this will be better breath. Decaying bacteria produce volatile sulphur compounds on the rear of the tongue.  These molecules account for about 85% of all cases of halitosis (bad breath). Scraping your tongue every night at bedtime can alleviate your bad breath.
